We are interested in determining how many volunteer hours are devoted to our
agency. This is something we have not tracked in the past, but would like to keep
statistics regarding volunteer hours.

Please use this form for your use in tracking the number of hours spent in the work of the disciplinary agency:

Pursuant to Rule 5.5d of the Louisiana Rules of Professional Conduct, a lawyer shall not employ, contract with as a consultant, engage as an independent contractor, or otherwise join in any other capacity, any person the attorney knows or reasonably should know is a suspended attorney during the period of suspension unless first preceded by the submission of a fully executed Employment Registration Statement to the Office of Disciplinary Counsel.
